Job Description :
Case manager needed for a large healthcare provider located in Logan, OH. Will assess, develop, review and carryout individualized service plans for consumers and families while securing and documenting consumer and custodial parent participation, maintain active caseload as assigned providing individual and group support treatment services, meet with consumers per agency policy and procedure, conduct telephone contacts and schedule home visits with required documentation in the consumer record, consult with clinical director as requested to review service delivery and planning, assist the individual in increasing social support networks (friends, family, community), and other general programming and reporting as needed.

This is a full-time, temporary-to-direct opportunity and if hired, this employer offers an outstanding benefits package including health, dental, vision, retirement, and more upon completion of the probationary period. The starting pay rate will be $10-12/hour, depending on qualifications. There might be an occasional need for weekend or evening hours, but the typical schedule will be Monday - Friday, 8:30 a.m. - 4:30 p.m.
Required Qualifications :

Seeking an individual with excellent communication skills as well as the ability to maintain clear and organized documentation. High school diploma or G.E.D. required but a degree in social work, psychology, or related human service field preferred. Must have 2 years field experience and experience with school-based services would be helpful. CDCA licensure or above preferred.
